The HUB CDC Programs & Services
The HUB CDC Programs & Services are managed through a proprietary case management solution which incorporates the tracking and measurement of member’s goals, sessions, objectives, assessments and plans. This secure web based system allows for case managers and associated partners to document their activities and communications with members, gives members a portal for self-reporting, evaluations and surveys and acts as a real time data solution for the overall management of member services.
The HUB Employment program prepares our members for the workforce through first providing an employment competency and skills assessment, identifying their strengths & weaknesses, ascertaining what their barriers are, completing a background check & drug screening and facilitating soft skills training. Once members are processed, we then link them with job fairs and other employment opportunities and resources.
